IMPORTANT INSTRUCTIONS FOR FILLING OUT
THE ITEM DONATION PAGE

Fill out the item name; estimated value, and a concise description
If you are donating the item, check "I am an individual donor"
If a business is donating the item, check "business donor"
List the name of the donor/donors as you want them to appear in the program
If you solicited this gift, enter your contact information in the Solicitor box: Name-Address-Phone-E-mail
Indicate your Drop-Off date preference from the drop-down box
Enter the DONOR information under the "Contact Information" section. If you are the solicitor and donor, enter your information only under the Contact Information section.

This information will go directly into our Greater Giving database, and a thank-you confirmation will automatically be sent electronically. Please be sure to include the donor's email on the form. Print off your Item Donation Form prior to submitting online. Please bring the completed form along with your auction item on our drop off Mondays & Tuesdays 4pm to 7pm held June 13, 14, 20, and 21 at Linnea Anderson's "Carriage House". The address is 13380 Rush Harbor Drive in Crosslake.
